I was driving through a shopping center parking lot the other day, and had to stop while an older guy limped from his car towards the tobacco store.
He had the look of a long-time smoker with COPD (IYKYK)ā¦.
and was limping with his entire leg from knee down wrapped in heavy bandages.
I couldnāt help but think he had probably just been discharged from the hospital (less than a mile away) after having a procedure done for ācritical limb ischemiaā. For anyone not familiar with this term, we have smokers in the hospital every day hospitalized with peripheral vascular disease. Many have a procedure called an angiogram to assess and sometimes bypass vascular areas of the legs due to damage from smoking.
These patients are told by physicians they need to stop smoking, or they will likely eventually need to have part of thwir leg amputated. It is still very common that they express no desire to stop smoking.

I have moderate pigmentary glaucoma, that is not responding with pressure decreases from a 2nd round of SLT laser treatment in 5 years. While my pressure ia stable, it is not near the 10 score my Opthamalogist wants (it ranges 14 to 19) every 3 month checkup) after SLT and new drops for a year. I already.have some peripheral vision loss in 1 eye, but see 20/20 with glasses drepite he and another Opthamologist literally giving me the āYou can still see 20/20 with that much nerve destroyed? Huh. You donāt have a lot of nerve left⦠Speech.
GLP-1 side effect? Retina damage in some, eyesight degradation in some.
Iām back in the Iām kind of freaking out about using it mode..Feel like I am more at risk and anything tipping the scales is super bad because I absolutely have to be able to drive for the next 10-15 years, work and comouter wise.

That is definitely a conundrum.
I typically would argue that losing the weight and protecting your cardiovascular health is pretty much always the top priority.
I also donāt have any idea how much risk is involved with your eyes, and the thought of losing eyesight is terrifying to me.

Have you had a calcium score/imaging done yet, and have you had your ApoB measured?
āApoB (Apolipoprotein B), which is a key blood test used to measure the number of harmful fat particles in your bloodstream that can contribute to cardiovascular disease and clogged arteries.ā
ApoB is probably the most important marker to measure. Thatās what I would attack first if elevated. I have other thoughts too, but out of the gate, based on what you described, I would cut back on saturated fat (You like Carnivore and KETO, right?) and eat more oilves and olive oil. Olives and Olive oil are proven to lower ApoB to a degree, but you may be a genetic super absorber and most likely need medication asap.
Longevity expert Peter Attia takes a combination of a PCSK9 inhibitor (such as Repatha/evolocumab) along with bempedoic acid and ezetimibe (often combined as the brand name drug Nexlizet). He uses this specific āstackā to aggressively lower his ApoB levels below 30 mg/dL without the potential side effects associated with statins.
Also, if you are taking Vitamin D without Magnesium and K2, stop that ASAP. Get on a really good fish oil ASAP. Probably 2 grams per day. One capsule per meal if you can tolerate that twice a day. And, only eat two meals per day. Walk after each meal, walk or work out while in fasted state in the morning. Working out while fasted burns more visceral fat. Per currrent āexpertsā.

Calcium score in 2019 2024 and 2025 as part of the Cardiac CT. 312 to 1165 to 1215. Big huge jump, then stabilized but still very very high ( 100 is top of desirable, 400 is high)
APOb is in range and ratio is in range. High end of normal.
LPa is elevated. Came down from 95 to 87 in a year. Testing again this year, needs to be like below 50. You hear plenty of chatter about how this is not modifiable and is genetic but it seems to very much be modifiable
D3 K2-MK7and 3 Magnesium blend in my stack with high DHA fish oil , Vit C, Turmeric, Lions Mane mushroom and CoQ10 been on them a long time. Allopurinol, Crestor Lisinopril HCZ and Jardiance. HS-CRP 1.1, CRP .1, so inflammatory stuff is low. Grounding bedsheet too. Inflammation is the enemy for me.
Literally while I was typing out some of these messages my better half was injecting herself with the first dose ever for her of Repatha

In asking here, Iām more interested in peopleās experience with conflicting goals in general and how they resolve them because so far Iām not super good at that in life across the board, not that itās easy and Iām at some kind of structural deficit just that tough decisions are tough decisions right?

Poorly, yes. Different things over the last couple decades where Iāve gone between 30 over and on target but never stable at the on target for more than a few years

This stuff for me is a small miracle going from 210 to 163 in just 16 weeksā¦
I have to force myself to eat nowā¦. But I simply use a timer on my cellā¦. I have to keep the protein intake going, water intake and lots of walkingā¦. Which is weird and slightly unexpected
I made sure I did an in person check uo monthly to have the doctor check me outā¦
Now that Iām in South Dakota for this job, Iām more in my own ⦠I have a script for three more sets but no increase in dosage so it will be four months on 10mg
Then I hope I can start reducing the dosages until Iām weened off
